Admission to Hidden Creek is meant to be an easy process for all. Step #1A resident or authorized representative needs to contact the Housing Director,Jennifer Cox at (952) 361-9338 extension 1, to begin the process. Step #2Cognitive and Physical assessments will be completed with perspective resident.We can perform these assessments in the perspective resident's current environment or on site during your tour of our home. Step#3Based on the assessment outcome, the Housing Director and nurse will determinewhat accommodations Hidden Creek can provide to meet resident's needs and the approximate cost. Step#4Prior to admission, all medications prescribed by Physician and any over the countermeds must be received. Step#5Family arranges time with Housing Director prior to the day the resident moves in.It works better for everyone if the admission paperwork is completed and furniture is in place before the resident moves in. |
